Fees: Make Checks Payable to AFI. Mail them to the office, along with your filled out form.

AFI ANNUAL RENEWAL FORM

Please complete this form and return to the above address along with your renewal dues by December 31^st. Married couples must file separate forms if both hold membership. Renewals postmarked after December 31^st will be charged a late fee. If your renewal is not received by January 15^th, your membership will be cancelled for non-compliance. It is legally required that all information is filled out. Please fill out every line in order to be legal. FORM MUST BE FILLED OUT IN ITS ENTIRETY.  RG
